OCHO Futsal Tryout Registration Form

Players are invited to attend our tryout and evaluation session. Our coaches will evaluate each player's current ability, understanding of the game, effort, and developmental needs to determine the most appropriate team and training environment.

Participation in a tryout does not guarantee team or tournament selection. Team placements and future opportunities are earned through consistent attendance, commitment, effort, availability, and performance.

5:00 PM: Boys 2017 & Girls 2016
6:30 PM: Boys 2014 & 2015

Tuesday (Just Soccer Futsal Center)

7:00–8:30 PM: Girls 2017

Wednesday (Just Soccer Futsal Center)

7:00–8:30 PM: Boys 2018 & 2019

Friday (Chaffey High School)

6:00–7:30 PM: Boys 2016 & Girls 2014 & 2015

I acknowledge and agree to the OCHO Futsal Liability Waiver and Release. I understand that participation in futsal and related activities involves inherent risks, including injury, and I voluntarily assume those risks. I release OCHO Futsal, its coaches, staff, and representatives from liability to the fullest extent permitted by law. If registering a minor, I confirm that I am the parent or legal guardian and have authority to agree on their behalf.
